Basic Information:
- Who they are (name)
- What they are (type of entity);
- Who is the owner (name, personal contact info);
- Where they are located (addresses);
- How to contact them (phone, fax, email, mobile phone);
- How they will order from you (purchase order, verbal, etc.);
- The amount and type of credit they are seeking (COD, Credit Card, Terms)
- Trade references;
- Bank reference.
Optional Additional Information
- Shipping address;
- Former addresses;
- List of all owners (name, title, social security #);
- Estimated annual sales;
- Type of business (manufacturer, wholesale, retail, internet retail, other);
- Mortgage holder or landlord contact information;
- Outstanding liens or judgments;
- Current or threatened litigation;
- Any NSF checks in last 6 months.

Basic Agreement Terms
- Explicitly states it is a binding agreement ;
- Person signing is authorized to bind company;
- The information is true;
- Vendor can evaluate Applicant’s credit;
- Vendor can evaluate owner's credit;
- Vendor’s discretion to grant credit;
- Applicant owes Vendor for purchases;
- Vendors forms have precedence;
Default Provisions
- Personal guaranty;
- Interest and acceleration;
- Collection costs and attorney fees;
- Litigate or Arbitrate;
- Vendors choice for venue, jurisdiction, and law;
- Jury waiver.

- Agree to provide financial statements
- Solvency Declaration
- Allocation of Payments
- Security Interests

- Separate for Personal Guaranty
